Measurement-based care fails on logistics, not intent.
Everyone agrees a PHQ-9 at every visit is good practice. In a busy clinic the form is handed out on paper, scored by hand if at all, and typed into the note when someone has time. The 96127 charge is missed more often than it is billed, and the scores never line up well enough to show a trend.
